2017-2018 CCOSA MEMBERSHIP FORM
The following steps will help you complete your form.
Please remember that if you are payroll deducting your dues, this form must be submitted to your district payroll clerk.renew_Modal
Step 1: Verify your information below making sure that all fields are completed and updated.
Step 2: Check the box indicating that all of your information is correct before proceeding to the next step.
Step 3: Choose your association. Your national membership is optional.
Step 4: Indicate your payment options:
- Payroll Deduction - You must submit your form to your district payroll clerk.
- Check (Payment in Full) - Print form and mail form with check to CCOSA.
- Credit Card - Pay Online
Step 5: Fill out beneficiary information, sign, date, and submit form. This does not apply to Associate Membership
